Lead Clinician/Clinical Manager

Turning Leaf’s Clinician is a Master’s level, licensed clinician that leads the multi-disciplinary team as it pertains to the therapeutic and treatment service components. The Clinician provides individual therapy, group therapy and works collaboratively on skill building programming and case management services for consumers ed pills. Responsible for monitoring the implementation and effectiveness of individual treatment and behavior plans. Responsible to communicate with and report to all stakeholders and community resources providing services to the consumer.    Experience with best practices such as Motivational Interviewing, Self Determination, Dialectical Behavior Therapy, Gentle Teaching, Trauma Informed Care, and experience with group therapy and individual CBT is highly preferred.

Health Care Coordinator

Turning Leaf’s Health Services Manager (HSM) is typically either an LPN or RN who oversees all general health and wellness, medical coordination, medication procurement and administration and overall coordination of behavioral and levitra medical care. Responsible for scheduling, monitoring and follow up with laboratory work per physician orders, and medical appointments (i.e. psychiatric, medical, podiatry, dental, vision, and neurological services). The HSM monitors medication administration while ensuring compliance with county, state and accreditation bodies standards.

Case Manager

Turning Leaf’s Case Manager is a bachelor’s level clinician who provides supports coordination and  case management services for consumers such as face-to-face contact and related activities including psychosocial assessment, Person Centered Planning, linking to services, utilizing natural/community supports, maximizing income/benefits and managing the overall plan of service. The Case Manager also coordinates with the multi-disciplinary team to assist with development, implementation, and maintenance of Behavior Support Plans. Facilitates treatment groups under the direction of the Lead Clinician; meets with consumers individually as needed to assess progress toward goals of treatment assist, guide, prompt, redirect & teach consumers skills to reach goals stated in their plan.

Community Integration Coordinator

Under the direction of the Lead Clinician/Clinical Director, the Community Integration Coordinator is the member of the multi-disciplinary team responsible for planning, coordination and implementation of Turning Leaf’s accredited Day Treatment and Community Integration programs. Facilitates and delivers evidence based therapies within a group setting, individualizes and modifies programming based on skill level.  Promotes community-based support services to include pursuit of education, volunteerism, and competitive employment when appropriate.

Program Manager/Assistant Program Manager

Turning Leaf’s Program Managers are members of the multidisciplinary team and operational leaders of the programs; providing positive role modeling for both consumers and direct care workers. Under supervision of Turning Leaf’s Director of Operations/Administrator, the Program Managers are responsible for the daily operations of the residential treatment and community housing programs. Program Managers are responsible for maintaining all daily operational standards of the programs including staffing, medication administration, documentation, communication with external stakeholders, participating in the person centered planning process and implementation of Turning Leaf Policies and Procedures. Program Managers meet regularly with consumers collectively and individually to assess progress towards treatment goals as well as to allow residents the opportunity to express issues and concerns. Program Managers directly supervise Assistant Program Managers, Shift Supervisors and Behavioral Health Technicians.

Behavioral Health Technicians

Behavioral Helath Technicians (BHT) work daily supporting consumers in their residential placement providing companionship, supervision, safety, personal care and community living supports. Essential members of the person-centered team, the BHTs inform the treatment process by documenting their interactions and observations of the consumers. BHTs ensure quality living standards are maintained as it relates to cleanliness, nutrition, and general health needs.  Under direction of Program Managers and Assistant Program Manager(s), BHTs assist the team by following treatment orders, administering medications, encouraging attendance at groups, promoting exercise, and providing transportation appointments while relaying pertinent information to the team.
