Admissions & Referrals | Turning Leaf Behavioral Health Services

Turning Leaf’s programs are for adults 18 years and older who require behaviorally-focused residential treatment due to a diagnosis of mental illness, developmental disability, co-occurring substance use disorder, personality disorder and/or history of traumatic brain injury.

Admission to the residential programs may be for the following diagnoses:

  • Mental Illness
  • Developmental Disability
  • Personality Disorder
  • Co-occurring Substance Use Disorders
  • History of Traumatic Brain Injury

Referrals to Turning Leaf may be initiated by:

  • Individual/applicant or guardian
  • Family member
  • Case manager
  • Physician
  • Mental health professional/Social worker
  • Discharge planner

OR: You can make the initial referral online. The process is as follows:

Step 1: Download the application forms.

Click on the three forms underscored below to download them to your computer for printing:

  • PDF iconPhysician Referral Form – The qualified mental health professional must complete this form and it should be mailed along with all pertinent supporting documents including lab results, psychiatric evaluations, psychosocial, progress notes, discharge summaries, etc.
  • PDF iconResident Application Form – The applicant is encouraged to complete this form.
  • PDF iconFinancial Form – The person or agency that will be financially responsible must complete this form.

Step 2: Complete the forms, sign and submit them.

Complete the three forms, attach the requested documents (e.g., recent medical records, reports and discharge summaries, plus photo copies of the applicant’s driver’s license, social security card, etc.). Include all signatures and mail or fax them to:

Step 3: Participate in an interview.

The consumer may be asked to participate in a pre-admission interview. This interview can be conducted in person at a Turning Leaf location, in a hospital or other behavioral health program. At the completion of the process, the Access Manager will inform the individual of his or her acceptance status.
