At Turning Leaf, our philosophy is to individualize each resident’s placement and therapy based on an evaluation conducted upon admission. We gather input from the resident, with assistance from the interdisciplinary team of case managers, guardians, family members and applicable health care professionals.
Psychosocial rehabilitation is the cornerstone of our residential programs. Our professional staff focuses on outcomes, not diagnoses (Please see below for program descriptions). By applying reality-based social and life-skill building therapies, whether vocational, recreational or daily living activities, we help our residents work through the environmental and/or interpersonal barriers that often keep them from living as self sufficiently as possible.
Turning Leaf’s residents are evaluated using our Skill Building program to determine their current skill levels. Our Skill Building program consists of six levels which address all aspects of independence. As our residents progress through the program, they are afforded greater levels of independence and are making steps towards moving to a less restrictive environment. Using the skill building program we establish goals and each resident is then placed in one of three different levels of care, based on his or her individualized person-centered plan:

When a resident is admitted to our program, he or she is placed in the skill building level which most appropriately matches the residents’ current skills, abilities and needs for further rehabilitation. Skill building groups are taught daily and the resident is responsible for practicing the skills between the time that the skill is presented and the time at which the evaluation is performed. Once a resident has completed the skills for a particular skill building group, he or she may be considered for promotion. The resident must demonstrate mastery of the skill set in which he/she is currently working through completion of an evaluation for promotion.
If a resident does not continue to use the skills learned, a recommendation may be made to place the resident back in a previous cottage to relearn and practice the necessary skills.

Elm Cottage (Level 1- Self Care Skills)
Elm Cottage is a 6 bed residential treatment program licensed as a specialized AFC. Residents living in Elm Cottage are focusing on basic activities of daily life and self care skills. In Elm Cottage skill building groups such as toileting, bathing, dressing and general hygiene assist residents in increasing their levels of independence so as to better prepare them for the steps to come.
Poplar Cottage (Level 2-Social Skills)
Poplar Cottage is a 6 bed residential treatment program licensed as a specialized AFC. Residents living In Poplar Cottage have mastered basic self care and are now working towards increasing the amounts of positive interactions with peers. Skill Building groups such as communication, problem solving, anger management, and relationships help the Residents of Poplar Cottage maintain appropriate social skills and relationships on the campus, in preparation for interacting with the community.
Redwood Cottage (Social Skills Cottage- Level 2 A)
Redwood Cottage is a 6 bed residential treatment program licensed as a specialized AFC. Redwood Cottage will open in fall of 2009 and focus on continuing social skills acquired in Poplar Cottage.
Maple Cottage (Level 3- Home Care Skills)
Maple Cottage is a 6 bed residential treatment program licensed as a specialized AFC. Residents living in Maple Cottage are using the skills learned in the previous level to appropriately interact in the rehabilitative environment. Skill Building groups in Maple Cottage focus on safety, home management activities (dishes, laundry, cooking etc), budgeting and shopping. Learning these skills will prepare the Maple Cottage Residents for greater levels of independence in the next cottage, which focuses on interacting with the community.
Cedar Cottage (Level 4- Community Skills)
Cedar is a 6 bed residential treatment program licensed as a specialized AFC. Residents living in Cedar Cottage are one step away from graduating to a more independent environment such as our Supported Independent Living Program. In Cedar Cottage the focus is shifting from succeeding in the rehabilitative environment to succeeding in a more independent setting. Skill Building courses focus on activities such as accessing community employment, navigating the community independently and accessing community transportation. Cedar Cottage residents participate in skill building groups, community outings, and recreational activities. Cedar Cottage Residents are continuing to work on vocational training onsite and are now encouraged to participate in community based vocational services, education and community support groups.
Northridge Home (MUSKEGON COUNTY)
Effective October 1st, 2009, Turning Leaf Residential Rehabilitation Services began operating Northridge Home. Northridge is a 7 bed residential program licensed as a specialized AFC and serves individuals challenged by mental illness. Residents in Northridge Home are functioning at various levels, and a modified version of the skill building program is available to all residents.
