Where is Turning Leaf located?
We are conveniently located in South Lansing, with attractive well-kept grounds. Situated on six acres, Turning Leaf provides a private park-like setting and a safe, relaxing environment for our residents. Our prime location allows us to easily interact with the community. We are located in close proximity to many local businesses, restaurants, and area attractions.
Who decides what services I need and how long I’ll be there?
Your treatment plan is developed based on your referring diagnosis with input from you, members of the interdisciplinary team and your family, when possible. Most of our services begin with an evaluation, followed by realistic goal setting strategies for improvement. Based on your evaluation, you will take part in classes as part of our Skill Building Program. Each class is designed to help you make progress towards your goals, and attain greater levels of independence. The actual length of service will depend on your individual needs and rate of progress.

What are the living arrangements?
Our specialized residential and transitional residential accommodations at Turning Leaf consist of spacious cottages with semi-private or private rooms. Each attached facility has its own laundry, kitchen, handicapped-accessible bathrooms and a large combined dining and living area, featuring pleasant and comfortable furnishings. We provide three home-cooked meals per day. Laundry services and medication assistance are also provided, based on individual needs. Our residential programs have staff onsite twenty-four hours a day.
Our supported independent living (SIP) program consists of a furnished two bedroom apartment setting. Residents in our SIP program have the opportunity to participate in activities on the Turning Leaf main campus, and staff assistance is provided as needed.
How are people referred to Turning Leaf?
We accept referrals from physicians, social workers, discharge planners, case managers and community mental health agencies. Clients or their families may also apply directly.
When can residents receive visitors?
Our residents can receive visitors at virtually any time. All visits should be arranged in advance with the social work department.
Can family members visit overnight?
We encourage family visits. Overnight visits can be arranged by contacting the social work department. Turning Leaf has an apartment available on site for families to use while visiting a loved one.
What do the residents do during the day?
Turning Leaf is now proud to offer a full day treatment program in addition to the individual counseling, nursing, or psychiatric services that are already being provided. The day treatment program is a new Structured Activity Program called “Get Up and G.O.E” the E’s stand for:
It consists of large group activities designed to complement the skills being learned in the Skill Building classes.
SKILL BUILDING AREAS OF FOCUS
Our new Skill Building Program consists of 6 domains, each with goals to encourage movement towards maximum independence. For levels 1-4, Turning Leaf’s Cottages have been designated to teach classes to reinforce the individual areas of focus in the Skill Building Program. Levels 5 and 6 begin in the cottages and are continually reinforced in our Supported Independent Living Program.
Residents participating in our Skill Building Program must apply to move within the levels. In order for the resident to be considered for promotion, he or she must demonstrate mastery of the skill set in which he/she is currently working. A Resident is eligible for evaluation when all of the skills have been presented to the resident in a group or a 1:1 setting. Resident is responsible for practice of these skills between the time that the skill is presented and the time at which the evaluation is preformed.
If residents do not continue using skills learned, a recommendation will be made to place residents back in a previous cottage to relearn and practice the necessary skills.

If I live in the Supported Independent Living Program, can I still participate in group activities?
Absolutely; Residents in our SIP program are encouraged to continue to participate in Day program, community inclusion outings. Your Turning Leaf case manager can work with you to create a schedule that meets your treatment needs.
