Admissions & Referrals

Turning Leaf provides rehabilitation programs for adults 18 years and older who require behaviorally-focused rehabilitative and therapeutic services in coping with a cognitive or mental impairment due to a traumatic brain injury, mental illness, Alzheimer’s or who are developmentally or physically challenged.  Our programs are designed to help our clients achieve and maintain a reality-based level of function and independence.

Admission to the residential programs for the following diagnoses is as follows:

  • Traumatic Brain Injury: Based on the findings of our assessment team.
  • Mental Illness: Based on a primary diagnosis, case manager’s evaluation and our own assessment.
  • Developmentally Disabled: Based on the findings of our assessment team.
  • Alzheimer’s Disease, or a related dementia: Based on physician and family referrals.

Referrals to Turning Leaf Rehabilitation may be initiated by a:

  • Case manager
  • Physician
  • Mental health professional/Social worker
  • Discharge planner
  • Individual/applicant
  • Family member

The admission process is as follows:

Step 1: Download the application forms.

Click on the three forms underscored below to download them to your computer for printing:

  • Physician Referral Form – The current attending physician must complete this form and it should be mailed along with all pertinent supporting documents including lab results, psychiatric evaluations, psychosocial, progress notes, discharge summaries, etc.
  • Resident Application Form – The applicant is encouraged to complete this form.
  • Financial Form – The person financially responsible must complete this form.

Step 2: Complete the forms, sign and submit them.

Complete the three forms, attach the requested documents (e.g., recent medical records, reports and discharge summaries, plus photo copies of the applicant’s driver’s license, social security card, etc.). Include all signatures and mail or fax them to:

Step 3: Participate in an interview.

The Admissions Coordinator will contact the applicant during the process and may ask the individual to participate in a pre-admission interview.

This interview can be conducted “in person” at Turning Leaf or in a hospital or mental health facility within a reasonable distance from Turning Leaf.

A final word:

The Turning Leaf Rehabilitation Assessment Team evaluates each applicant for potential admission. At the completion of the process, the Admissions Coordinator will inform the individual of his or her acceptance status.
